THE WEBINAR
The application of laser therapy as an adjunct in gynaecological diseases has increased considerably. Many specialists now consider introducing laser technology into their gynaecology and outpatient procedures. We are convinced of its benefits and so, have commenced our laser workshops on gynaecological surgery.

The basic intention is to provide a practical introduction and approach to technical issues.

LEFT MONOLATERAL ENDOMETRIOMA +PELVIC ADHESIONS: C02 FIBER LASER VAPORIZATION +ADHESIOLYSIS +DIAGNOSTIC HYSTEROSCOPY

Present complaint: dysmenorrhea and chronic pelvic pain.

TV-US (30th May 2019):

  • Uterus: retroverted, mobile, with posterior adenomyosis.
  • Endometrium: regular
  • Right adnexa: regular, hypomobile.
  • Left adnexa: fixed, increased in volume due to the presence of: Endometrioma: 35x49mm
  • Recto-vaginal pouch: no lesions
  • Vescico-uterine pouch: no lesions

Physical examination (30th May 2019):

  • External genitalia: nulliparous
  • Vagina: regular
  • Uterus: anteverted, hypomobile.
  • Right ovary: hypomobile, regular
  • Left ovary: fixed, increased in volume

Remote history:

  • Obstetrical history: Gravida 0
  • Previous surgery: LPS (2013): right endometrioma enucleation + adhesiolysis
  • Hormonal Therapy: Continuous P therapy

When do you operate for patients with endometriosis and require fertility? We see patients with recurrent endometrioma after surgery and now need IVF. Would you use size of endometrioma to decide if the patient needs surgery before IVF?

How about sclerotherapy? Could it be an alternative to thermal ablation or excision?

This is not the right treatment. The endometriomas will return within a year with ablation.

How about sclerotherapy? Could it be an alternative to thermal ablation or excision and whate is best for surgery.. Its simple cystotomy or with lazer abalation?
